He was predeceased by: his parents, James Chizick and Helen Chizick (Donitzen); his sister Charlotte Mayfield; and his brothers-in-law, James McHale, Robert Mayfield and Thomas Wiedrick.
He is survived by: his former wife Joy; his wife Joyce (Vertichio); his children, Jonathan Chizick (Tristan), Jenée Chizick Agüero (Arturo), Jarett Chizick and Jordan Chizick (Raquel); his grandchildren, Braelan, Ridley, Larsen, Yohel, Jacek, Finn, Livia and Lucas; and his sisters, Helen McHale, Barbara Mirabito (Paul), Christine Vollmar (Gary) and Cynthia Wiedrick.
