Azzolino (Carrie), Mark J. Azzolino (Jennifer) and Suzanne O'Neil (David Carleo); his grandchildren, Ethan, Sean, Natalie, Veronica, Ryan and Hank; his brother Thomas Azzolino (Mary); and his sister-in-law Mary Anne Azzolino. He is also survived by several nieces, nephews, cousins, and many special friends.
