He was predeceased by: his parents, Luigi Evangelista and Maria Teresa Cardillo Evangelista; and his wife Elena Evangelista. He is survived by: his daughters, Teresa Couitt (Gary) of Liberty, NY and Maria Cherepanov (Steve) of Liberty, NY; his granddaughter Megan Couitt of Liberty, NY; and his great grandchildren, Izek Resto and Kyrie Hunter.

Memorial contributions in his name may be made to Achieve Rehab. and Nursing Facility, General Patient's Fund, 170 Lake Street, Liberty, NY 12754 or a charity of one's choice.
