He was predeceased by: his parents, Alvino and Rose. He is survived by: his wife Susan; his children, Lisa, Anthony (Kimberly) and Gina; his siblings, Richard (Ann), Eileen and Robert (Sheila); and his granddaughter Gloria Rose. He is also survived by numerous nephews and nieces.

In lieu of flowers, donations can be made in David's name to St. Jude's Children's Research Hospital 501 St. Jude Place, Memphis, TN 38105.
