He is survived by: his parents, Philip Formica and Lynn Formica (Schabot) of Lake Katrine; his siblings, Aris Henderson of Saugerties and Jamie Ladd (Kim) of Catskill; his grandparents, Enrico and Claire Formica of Albany, NY; his nephews, Colton and Boden Ladd of Catskill; his niece Adrianna Alexander of Saugerties; his aunts and uncles, Mary (Charlie Redlich), Pete (Pam Formica), Margaret (Rob Muenkel), Ann (Tom Krause), Sarah Formica (Jason Varnell), Sue (Dan Nelson), Patty (Mike Sawick), Bill (Amy Schabot) and Bruce LaTorre; and his cousins, Jessica Maggiore (Shawn), Sara LaTorre, Eric, Jeff Nelson, Nick, Marissa Sawick, Bri, Dan Muenkel, Jeremy, Sam Redlich, Gabby and Zach Formica.
