He was predeceased by: his parents, Kermie Cook and Marilyn Gilchrist Cook. He is survived by: his wife Lillian Cottrill Cook; his children, Mindy Rutledge, Ken, Michael S. Cook (Amy), Brian Cook (Becky) and Jennifer Leon (Jason); and his siblings, Michelle Green, Jeffrey Cook, Mark Cook and Gregory Cook. He is also survived by several grandchildren, great grandchildren, nieces and nephews.
