He was predeceased by: his father George F. Maye Sr.; and his niece Lorianna. He is survived by: his significant other Lissette Majewski; his mother Mary Maye; his siblings, Laurie Murray (Harv), George Maye Jr. (Annette) and Corey Maye (Brandi); his children, Chad Jr., Jared and Georgie Maye; and his nieces and nephews, Austin Maye, David Ward Jr., Sena Darling, Breanna Maye, Corey-Michael Maye and Easton Prue.
In lieu of flowers, memorial donations in Chads memory may be made to Dolgeville Forward C.A.T. project.
