He was predeceased by: his wife Elizabeth A. Cook (Melkulcok); his parents, Harold G. Cook Sr. and Clara Cook (Valentine); his siblings, Larry Cook and Carol Cook; and his daughter-in-law Sherry Cook (Miller).
He is survived by: his children, Harold R. Cook Jr., Jacqueline Lindsley (Cook) and John Cook (Wendy); his grandchildren, Bill (Kate), Stephen (Nannette), Ashley, Kristine (Matt), Kyle, Tyler and Courtney (Tony); and his great grandchildren, Wesley, Sophia and Addison. He is also survived by many cousins, nieces, and nephews.
