He was predeceased by: his parents, Frederick Hayes Sr. and Virginia Dare (Jean); and his former wife JoElla Palmer Hayes. He is survived by: his son Fred O. Hayes III (Sarah) of Ontario; and his grandchildren, Susan and Shaina.
