He was predeceased by: his parents, Herbert M Cook Sr. and Helen Van Cook; his wife Nancy P Cook; his son David O Cook; and his siblings, Robert W Cook and Shirley A Breen.
He is survived by: his brother Herbert M Cook Jr. (Sandra T Cook) of Ames; and his sibling-in-law Ithaca. He is also survived by many nieces and nephews.
