He was predeceased by: his parents, Ben Lewis and Virginia Lewis (Eddy); his sons, Neil and Erik; and his sister Deborah.
He is survived by: his wife Charlene; his sons, Ryan of Ilion and Kevin (Jenny) of Mohawk; his siblings-in-law, Maureen (Dave Chisholm), George (Jayne) of Oneida, Shireen and Coleen Wentworth of Oneida; his grandchildren, Kara and Nicholas; and his cousin Charles Thomes.
In lieu of flowers, Dale's family request donations to the Herkimer County Humane Society or the American Diabetes Association, envelopes are available at the funeral home.
