He was predeceased by: his parents, Jack Salvatore Sr. and Elizabeth Osterhoudt Salvatore; his brother Butch Salvatore; and his niece Lauren Kelly Minard. He is survived by: his wife Judy C. Critchley; his children, Jayson Salvatore of Poughkeepsie, Erin Petronella of Poughkeepsie and Jack Salvatore III of Poughkeepsie; his stepdaughter Sherri Levy of Nashville, TN.; his grandchildren, Keilee, Aaliyah Salvatore, John Thomas and Shay Petronella; his sister Dee Minard (Darren) of Poughkeepsie; his niece Danielle Minard; his great-nieces and great-nephews, Kasen and Alex Minard (Amanda); and also, Alex Jr., Brody, Sean Minard, Camden, Lincoln and Rowen.
