He was predeceased by: his parents, Irene Cook and Delos Cook; his siblings, Bob Cook (Rose) and Betty Galka (Stanley); and his children, Richard Cook (Melinda) and Patricia Cook.
He is survived by: his wife Gladys Cook; his siblings, Patty Aubrey (Gordon), Janet Pagano (John) and Ronald Cook (Lucy); and his children, Debra Cahill (John), Jeffrey Cook (Kay), Scott Cook (Karrie) and Randy Cook (Cindy). He is also survived by 12 grandchildren, 24 great grandchildren and many great-great grandchildren; and several nieces, nephews and cousins.
