He was predeceased by: his mother Emily Deugaw; his stepfather Paul R. Boak; his son Gary Michael Boak (Mary); his sister Josephine VanDyke (Paul); and his stepbrothers, Lawrence and Paul M. Boak.

He is survived by: his wife Delores Morehead; his sons, Jack (Jay) and Jerry Boak (Tina); his granddaughter Emily Moylan (Peter); his great grandchildren, Elle, Isla of Norwood, NJ, Dr. Nicholas Boak (Thomas), Alison Weinstien (Allan), Michelle Shaaban (Mohamed) and Melissa Vasquez (Eleazar); his daughter-in-law Mary Boak Bidwell (Neil); his grandson-in-law Daniel of Yorkville, NY.; his nephews, Guy VanDyke of Louisville, KY and Terry Boak (Patricia) of Holland Patent, NY.; and also, Sloane of Atlanta, GA, Elena, Lidia, Ella, Alexandra of Pound Ridge, NY, Mazin of Concord, Quinn and Emmett of Ocoee, FL.

In lieu of flowers remembrances in Jack's name may be made to the Leon R. Roberts Post 161 American Legion of Holland Patent or the Holland Patent Volunteer Fire Dept.
