He was predeceased by: his parents, Leo E. Forness and Marie Forness (Hassett); his son Patrick Forness; his granddaughter Jessica Hanchett; and his sisters, Patricia Wilber and Ann Pezzimenti.
He is survived by: his wife Marilyn K. Smith (Tom); his children, Christopher J. Forness (Jamie) of Wellsville, Kelly Goodridge of Wellsville and Lisa Forness of Whitesville; his daughter-in-law Kim Forness of Troy, Ohio; his grandchildren, Katelyn Waters (Cory), Joshua Hanchett (Courtney Buentello), Matthew Forness, Megan Goodridge (Kade Hanchett), Courtney Ainsworth (C. J. Knight), Kendra Forness (Jerrod Gullett), Thomas W. Forness II, Gabrielle Forness, Grace Forness, Serena Ainsworth, Brennan Forness and Camron Lyons; and his brothers, Edward Forness and Ray Forness. He is also survived by several nieces and nephews.
