He was predeceased by: his parents, George Cook and Dorothy Cook (Blackman); his wife Jane Pogonowski of John's Church; and his siblings, Paul, Harold Cook and Eleanor Miller.
He is survived by: his children, Wayne Cook and Diane Cook of Newport; his brother Jack Cook (Patricia) of Cleveland; his grandson Dustin Cook (Katie) of Conroe, TX; and his great grandchildren, Zoe and Aria Cook. He is also survived by several nieces and nephews.
