He was predeceased by: his parents, Anthony J. Hlavaty and Emily C. Roach Hlavaty; his wife Margaret M. Murphy; and his sons, Bobby and Raymond.
He is survived by: his children, Paul of Las Vegas, NV, Barbara O'Connor (Greg) of Monroe, NY, Kathy Rozier of Sitka, Ala and Peg Hlavaty of Pueblo, CO.; and his sisters, Helene Tallo (Gerry) of Riverhead, NY and Lois Piscopo (Al) of Peekskill, NY.. He is also survived by his eleven grandchildren and five great-grandchildren.
