He is survived by: his spouse Kathy Church; his sons, Mathew Wood, Noah Evans and Brian Church; his mother Dawn Patricia Philo Church; and his siblings, Church Case, Cindy Church Williams, Amy Church, Ricky Church and George Church. He is also survived by eleven grandchildren, several nieces, nephews, and cousins.
