He is survived by: his wife Meghan Phalen; his children, Caitlin Johnston, Joshua Johnston (Christine) and Erin Davis; his adoptive children, Alexis Skarrup, Sweet Rudy Blue Eyes and Amandahugandkiss; his grandchildren, Fallon and Parker Davis; his parents, Wayne Perkins and Barb Perkins; his brothers, Pete Phalen (Teasa) and Jeremy Perkins (Erin); his aunts and uncles, Barry Phalen, Rick Dixon (Joyce), Sandy Johnson (Terry) and Danielle Cifone; his cousin Jennifer Wilcox (Steve); and his parents-in-law, Dan, Judy Shaw and Aub Shaw (Victoria Voelkner).
