She was predeceased by: her parent Corliss M. Harvey (Forster). She is survived by: her father Harold W. Harvey; her husband Thomas J. Chupa; her sons, Nicholas W. Chupa of Dayton and Nathan T. Chupa (Erica Lecceardone) of Dayton; and her sister Crystal Henderson (Jerry) of South Dayton.
