He was predeceased by: his father Carlton W. Forsythe. He is survived by: his wife Karen Butler; his daughters, Kimberly (Kenneth Weeks) and Kathy (Timothy Drake); and his siblings, Carlton Forsythe Jr. of Gouverneur, Carlene Forsythe of Gouverneur and Janice Forsythe of Dekalb. He is also survived by six grandchildren, five great-grandchildren and several nieces and nephews.
