He was predeceased by: his parents, Frederic F. LaVack and June E. LaVack (Washburn); his siblings, Alton LaVack, Mary Ritter and Judy LaVack; and his pet Cocoa.

He is survived by: his sister Susan Narog. He is also survived by several nieces, nephews, aunts and uncles.

Donations in memory of Dave may be made to Friends 4 Pound Paws or the Gouverneur Rescue Squad.
