He is survived by: his parents, Raymond Cook and Shirley Curl Cook; his daughters, Kerri Cook Viglione and Kacey Cook Mann; his grandchildren, Madeline, Brandon, Gabriel, Francesca and Giuseppe; his siblings, Kathy Kingston, Shelia DuBois, Paula Adams, Kenneth Goodale, Nancy Villarreal, Wesley Goodale, Linda Monroe, Dawn Rossi, Mary Quartiers, Sue Goodale Berkowitz and Hollis Goodale Jr; and his close friend Victor Mhyrberg.
