He is survived by: his wife Shelly; his children, Marissa, Craig (Lauren), Shane and Ryan; his grandchildren, Jason, Fiona and Silas; his parents, Calvin Cook and Fanny Cook (Mackey); and his siblings, Roy Cook (Teresa), Terry Cook (Claudia) and Melinda Acri (Martin). He is also survived by many nieces, nephews, cousins, aunts and uncles.
