He is survived by: his parents, Tina Wallace (Mastro) of Hannibal and Scott Cook, Sr. (Betty) of Fulton; his fiancé Billie Jo Donahue of Fulton; his sons, Scott III and Mason Cook; his siblings, Tasha Whalen (Tyler) of Fulton, Dustin Cook (Andrea) of Oswego, Desteney Cook (Ryan Handville) of Fulton, Katherine Victory (Josh) of Fulton, Dale Richardson of Fulton, Amber Smith (Josh) of Palermo, Zachary Cook (Dionna Pepper) of Fulton, David Mastro of Hannibal, John Wallace (Helen) of Greece and David Wallace (Melissa) of Rochester; and his grandparents, Frank, Marilyn Mastro of Lowville, Janis Cook of Fulton, Dan and Darlene Wallace of Florida.
