He was predeceased by: his daughter Elizabeth. He is survived by: his brother Bruce; his wife Lisa Maheu Hawthorne; his daughter Olivia; and his siblings, Dennis Sr., Mark and Joyce Cook. He is also survived by many nieces and nephews who grew up together like brothers and sisters.

In lieu of flowers, contributions can be made to The Transplant House of Cleveland, Sterling Nature Center or your favorite animal rescue.
