He is survived by: his wife Monica Woolworth; his sons, Matt Smith (Amie), Michael Smith and Mitchell Smith; his grandchildren, Nicholas Smith, Mikaylee Vail, Madelynn Smith, Trevor Smith, Cody Richardson, Jenna Richardson and Claudia Chetney; his mother Sandra Woolworth; his siblings, Dawn Woolworth and Karl Woolworth (Beth); and his parents-in-law, Charles and Madeline Smith.
