He was predeceased by: his father Michael Cook, Sr.. He is survived by: his mother Deborah Floyd Bentz; his wife Jill Cook; his sons, James Cook and Nathan Cook; and his brothers, Michael P Cook, Jr. (Misty) and Robert Cook. He is also survived by several nieces, nephews, and cousins.
