He was predeceased by: his parents, John Ellsworth and Jean Collins Ellsworth; his sister Janet Ellsworth; and his nephews, Aaron and Seth.

He is survived by: his siblings, Donald Ellsworth (Kathy) of Fort Covington, David Ellsworth (Elizabeth) of Tulsa and Mary Beth Premo of Massena, N. Y.; and his nieces and nephews, Matthew (Kassaundra), Steffanie, Jeremy, Andrew (Stacy), Beth, Amie (Jeremy), Daniel and Michael (Laurel).
