He was predeceased by: his parents, David Nash and Elizabeth Marie Fossum (Funk); and his siblings, Pam Mathern and Perry Ellsworth. He is survived by: his wife Tamara Ann Hamman Ellsworth; his daughters, Megan Ellsworth of North East and Chandra Struble of North East; his grandchildren, Kallasia Ellsworth (Derek) and Kaiden Ellsworth; his siblings, Jan Plath (Jim) of Minnesota, DeeDee Ellsworth of Fargo, N. D. and Richard Ellsworth (Margie) of Fargo, N. D.; and his half-brother Matt Fossum (Geri).
