She was predeceased by: her parents, David Phelps and Jessica Kozich; and her husband James G. Forbes. She is survived by: her daughters, Karen Blackman (Jim Rodgers), Eileen (Kyle Konecny) and Dianne (Nick Monico); her grandchildren, Jessica Blackman (Steve Gofran), Tyler (Katrina Plew), Kirk (Betsy Blackman), Dylan Konecny (Darian Meagley) and David Monico (Sarah Espe); and her great grandchildren, Addison, Aliana Blackman, Adrienne Exley, Wyatt Plew, Lucas and Sylvia Blackman. She is also survived by several nieces and nephews.
