He was predeceased by: his father Thomas Bates; and his brother Thomas Norman Bates. He is survived by: his wife Gloria (Worley); his daughter Jennifer Greenwald; and his sister Caroline Woodworth. He is also survived by nieces, nephews and cousins.
In lieu of flowers, contributions may be made in Dave's memory to the Alzheimer's Association.
