He was predeceased by: his parents, Angelo Formisano and Frieda Formisano (Damiano). He is survived by: his wife Judy Formisano (Valle); his son Philip P. Formisano; his grandson Philip P. Formisano Jr.; and his siblings, Anthony Formisano and Maria Klatt.
