He was predeceased by: his former wife Donna. He is survived by: his wife Suzanne M.; his children, Amy Shannon (Thomas), Daniel Thomas Bouza II (Joy), Johnathan and Kevin (Ashley); his stepsons, Jason and Anthony; his grandchildren, Parker, Londyn and Holland; and his siblings, Gabrielle Bradley (Skip), Joseph (Pam) and Philip (Karen).
