He is survived by: his wife Jean (Cusmano); his daughters, April Kozlowski (Kenny) and Dawn Cicio; his grandson Kyle Kozlowski; his brother Rudy Mendez; and his caregivers, Michelle, Madison and Jamie. He is also survived by many relatives and in-laws.
