He was predeceased by: his siblings, Mariano, Marion "Doc", John, Tony Forti, Teresa Hohl, Angeline Antonucci, Mary Kraft, Rose Leberman, Sam, Joe and Charles Forti.
He is survived by: his wife Carol Forti (Buerger); his children, James A. Forti (Mary Ellen), Jean Forti and Julie Fregelette (Jim); his grandchildren, Katherine, Michael (Kelsey) and James; and his close friends, Riley and Yorkie.
In lieu of flowers if so, desired donations can be made to St. Jude in Jim memory.
