She was predeceased by: her parents, Kermit Forman and Mildred Forman (Scott); and her sons, Timothy and Scott. She is survived by: her grandchildren, Adam, Eric, Cody, Derek, Emily Burns, Sidney Rider and Ashley Stone; her daughter-in-law Laurie Burns; her great grandchildren, Izabell, Beau, Mack, Jameson Burns and Gavin Rider; her sister Mary Inman; and her close friend Dan Sheldon. She is also survived by several nieces, nephews and cousins.
