He was predeceased by: his parents, Ralph G. Costanzo and Mary Costanzo (Santomassino); his siblings, Joseph Costanzo and Roseanne (Frank Krisch); and his brother-in-law Anthony Cavallo.

He is survived by: his wife Lucille Cavallo; his sister-in-law Betty Cavallo; and his nieces and nephews, Alicia (Ted Hughes), Tony (Kara Cavallo), Amy (Clint Barkdoll), Anthony Costanzo, Chris (Mona Costanzo), MaryAnn (John Costabile) and Joe (Laurie Costanzo).

Memorial Contributions in Ralph's name may be made to the Storm King Engine Fire Company in Cornwall on Hudson, NY 12520.
