He was predeceased by: his parents, Robert Cook and Ruth Cook (Mourhess); his wife Phyllis Wise; and his sisters, Margaret (Joseph 'Chic' Bamonti) of Corning and Geraldine (James Dandrea) of Elmira.
He is survived by: his son Todd (Marilyn 'Missy' Hopper) of Painted Post; his nephew Phillip Bamonti (Barbara) of Simpsonville, SC; his nieces, Mary Walsh (late Mike), Jo Ann Hill of Buffalo, NY, Roberta Hopkins (Bart) of Rochester, NY, Margaret Buscaglia (Joe) of Buffalo, NY and Ruth Dandrea of Utica, NY; and his aunt Helen Mourhess of Hanover, PA.
