He was predeceased by: his parents, John Hoffman and Virginia Hoffman (Clark). He is survived by: his wife Joyce Hoffman (Whell); his children, Jason, Tonya, Terry and Brian; and his uncle Sam. He is also survived by 13 grandchildren and many cousins and friends.
In lieu of flowers, memorial donations may be made to the Troy Police Benevolent Association in Jack's name.
