While his presence may be gone, his impact lives on in the kindness shown to others and the joy found in the ordinary moments of life.
He was predeceased by: his parents, Paul and Wilma. He is survived by: his daughters, Caitlin Costanzo and Olivia Duckworth; his grandson Vincent Costanzo; his sister Christine Patrick; and his nephews, David Patrick and Stephen Patrick. He is also survived by friends, family, and neighbors.
In lieu of flowers, the family requests donations be made to the Crohn's and Colitis Foundation or the Arbor Day Foundation in David's memory.
