He was predeceased by: his wife Renee Licata Forish; and his grandparents, Charles and Florence Matijas. He is survived by: his mother Cynthia Forish Czubat; his stepson Jae Simo; his sister Amy Jo Kaufman; his niece Rachel Kaufman; his nephew Josef Kaufman; his close friends, Michael Gauthier and Mike "Hokey" Volpe; his pets, Ray and Layla; and also Oliver Twist.
Visitation will be held on Monday, December 2nd 2024 from 10:00 AM to 11:45 AM at the St. Mary's Church (17 Clark St, Auburn, NY 13021). A funeral Mass will be held on Monday, December 2nd 2024 at 12:00 PM at the same location.
Contributions may be made in memory of Jason to St. Mary's Church, Auburn or the Finger Lakes SPCA of CNY.
