He is survived by: his mother Cynthia Forish Czubat; his stepson Jae Simo; his sister Amy Jo Kaufman; his niece Rachel Kaufman; his nephew Josef Kaufman; his close friends, Michael Gauthier and Mike "Hokey" Volpe; his pets, Ray and Layla; and also Oliver Twist.
