He is survived by: his wife Kathleen Joan Hoffman (Clark); his son Mark Forrest Hoffman (Donna) of Circleville; his sister Dorothy Cook of Circleville; his son-in-law Donald Balthaser of Ashville; his grandchildren, Beth Yeazel (Brian) of Washington, Jenna Gilbert (Dane) of Waverly, Adam Balthaser and Ben Balthaser; and his stepgrandchildren, Steven Hardbarger (Heather) of Logan and Stacey Thomas (Daniel) of Circleville.
