He was predeceased by: his parents, Joseph Sr. Polach and Marion Polach; and his brothers, Steven William Polach and Barry Lee Polach. He is survived by: his brother Joseph Polach Jr.; his sister-in-law Kathy; his sister Julie Fulling; his brother-in-law Robert; his nieces and nephews, Darian, Joseph, Steven, Sarah and Cullen; and his close friend Michael Carpinello. He is also survived by aunts, uncles, cousins and friends and his four-legged friend Buddy.
