He was predeceased by: his parents, Ann Azzolino and Anthony Azzolino; and his siblings, Toni Ann Azzolino and Anthony Azzolino. He is survived by: his wife Stephanie Azzolino (Bo); his son Nicholas Azzolino (Jennifer Giordano); and his sisters, Kathy Lynch, Debbie Harilla (Joe) and Marie Meehan.
