He was predeceased by: his parents, Charles Lewis and Amy Lewis (Noyes); his former wife Mary Lewis; and his siblings, Caroline Hardy and Charles Lewis.
He is survived by: his wife Marilyn Lewis (Belokur); his children, Paula (Stephen Juhola), David, Jr. (Janice Lewis), Earl "Josh" Lewis and Nathan (Gillian Lewis); his children-in-law, Andrew, Cindy Belokur, Kevin Vought and Revs; and his brother-in-law Floyd Jayne.
