He was predeceased by: his parents, Andrea Provino and Rosalia Provino (D'Amico); his son Dominick (Irene); and his siblings, Frances and Michael.
He is survived by: his children, Katherine Hilborn (Ralph) and Anthony Provino (Darlene); his daughter-in-law Mariana (Dominick); his sister Elizabeth Madeo; and his sisters-in-law, Stella Franciosa and Mary Provino (Christine Stangl). He is also survived by eight grandchildren and 10 great grandchildren.
In lieu of flowers, donations can be made in his memory to St.Jude Children's Hospital.
