He was predeceased by: his daughter Karen Chapman; and his sister Nancy Clark. He is survived by: his wife Jeanette Chapman (Hummel); his children, Douglas Chapman (Mary), Jeanette Kuznik (Paul) and William; his grandchildren, Paul, Kaitlyn, Ryan, Collin, Austin, Nathan, Gavin Kuznik, Matthew, Melissa and Sara Chapman; his great grandchildren, Jase and Brooklyn; and his siblings, Barbara Hartman (Paul), Samuel Chapman (Donna), Jane Enser and Ronald Chapman. He is also survived by many nieces and nephews.

In lieu of flowers, donations in Doug's name may be made to Roswell Park Alliance Foundation or Erie County SPCA.
