He is survived by: his wife Starr R. Mazur; his children, Kad R. Sullivan and Brooklyn J. Sullivan of Constableville; and his siblings, Rebecca Sullivan, Steven Sullivan (Dawn), Margaret Krokowski (Sullivan), Tracy Hastwell, III (John), Jeffrey Sullivan (Sheila) and Janet Evans Brantingham (Matthew). He is also survived by many nieces and nephews.
