He is survived by: his children, Caitlin, Ryan and Grace (Christine); and his siblings, Dennis Cusack, Michael Cusack (Marla), Kevin Cusack (Carol), Mary White, Kerry Morehead (Eric) and Erin Jones (Todd). He is also survived by many nieces and nephews.
